Simply contact our office at 509-383-4325 and we will answer any remaining questions you may have. New/Established visits are by appointment only.

New/Established Patients

New Patients; 1) Call 509-383-4325 for initial new patient screening. 2) If approved, complete applicable patient packet and demographics form. Email or mail with a copy of driver’s license and insurance card (front/back) and mail or email to billing dept below.

New/Established patients; Complete your online check-in (*email reminder with check-in link arrives 7 days prior to visit) and establish a Practice Fusion account for portal access. Check-in, review and update information prior to each visit. There is a short quiz that helps us understand your mood, complete this prior to each visit.

***Remember it is the patient responsibility to keep the provider and billing department aware of any changes in address or insurance information. It is also the patient responsibility to contact their insurance for eligibility of benefits, preferred provider status or need to authorize benefits/visits. A patient who neglects to update information or verify/update benefit status may be responsible to pay private pay rates. Thank you.
